There's no single Veterans Affairs fax number for medical records. Each VA Medical Center has its own Release of Information (ROI) office and fax number.

To request your records:

1. Complete VA Form 10-5345a. Fill out VA Form 10-5345a, which tells the VA what health information you are requesting and how you want to receive it. 

2. Find your VA health facility. Use the VA Facility Locator to search for the facility where you received care. 

3. Open the facility's website. From the facility page, select the link to its VA health care website. 

4. Go to “Other services.” On the facility's website, find Other services, then select Access your health records. 

5. Find the Release of Information office. On the facility’s medical records page, look for “Get Your Records by Fax” or “Find a Phone Number.” You can also press Ctrl + F and search for “fax” to quickly find the fax number on the page.

6. Fax your completed form. Send VA Form 10-5345a to the listed fax number. The office will process your request and provide your records using the delivery method you selected on the form.

When faxing the VA, use a fax cover sheet to help match your documents to the correct file.

Include the following:

• Full name

• VA file number

• Total page count
